Family-Centered Medical Home in Pediatric Care

I found a great article that speaks well and to the point about a framework of family-centered medical home in pediatric care.

For families that have a child with special health care needs, it is so critical for these families to have access to comprehensive care coordination as part of their medical home services. Care coordination helps families address four main components of the child and family's life; physical health, mental health, social health determinants, and the beliefs and practices of the family.
